The 'CKD Ambassador Program' is a Kidney Health Australia initiative providing an opportunity to implement best practice detection and management of Chronic Kidney Disease (CKD) with reward and recognition as an Ambassador.

Outcomes: Primary Care Health Professionals are:

  1. Supported in implementing best-practice systems to detect and manage CKD in high-risk individuals in their clinical practice
  2. Provided access to tools and resources to provide best-practice CKD detection and management in high risk individuals in their clinical practice
  3. Recognised as a CKD Ambassador amongst their peers and patients as providing best practice detection and management of CKD

How long does it take?

It is anticipated that the program will take 3-6 months to complete. If your practice is ready to begin the PDSA cycle you should be able to collect final results in time for Kidney Health Week 2019.

How to become an CKD Ambassador

To become an Ambassador, GPs, Practice Nurses, Aboriginal Health Workers (and other relevant primary care health professionals) need to:

1. Complete the Application Form

2. Undertake the CKD Ambassador Prep activity

3. Collect baseline data and complete the pre-disposing activity

4. Complete Plan-Do-Study-Act cycles to detect CKD

5. Submit final data and complete the reflection activity.

Participants who complete all parts of the program will have the opportunity to be a Kidney Health Australia endorsed CKD Ambassador. You will also be awarded CPD points, a certificate for each individual and a certificate for the practice (if involved at a Practice level).
