Our Kidney Youth Program helps young people transition from child to adult care through support, connection and resources.
For young people living with kidney disease, moving from paediatric to adult health care can be a difficult period to navigate. Additionally, living with kidney disease can be isolating. It can be hard to find people to talk to that understand what it means to live with kidney disease.
To help young people create meaningful connections with people that understand what they are going through, we have a number of resources for young people including a Facebook Group, Panel Discussions with our Kidney Youth Champions, and helpful documents.
Our Facebook Group is a safe place to connect, socialise, and run events. It is moderated by Kidney Health Australia staff and trained volunteers who also live with kidney disease; we call them our Kidney Youth Champions.
Our Kidney Youth Champions are volunteer mentors here to help other young people living with kidney disease. They have shared their experiences with kidney disease. Watch here.
